1. You have received a pre-filled request form from your general practitioner.
2. The order sticker (with barcode) is attached to the request form.
3. Complete the request form with date and time of sampling.
4. Make sure that your name, date of birth and sampling site (V=Vaginal / A=Anal / O=Oral-Throat) are recorded on each tube and place a sticker with barcode on the tube.
5. Collect your sample according to the packaging insert.
6. Open the envelope labelled MEDICAL POST and remove the carton and safety bag.
7. Ensure that the cap of each tube is close properly and place the tube(s) in the plastic safety bag and seal the bag by removing the protective strip on the blue strip.
8. Insert the sealed safety bag with tubes into the carton for protection and insert the entire package in the return envelope labelled MEDICAL POST Star-shl.
9. Place the request form received from your GP in the same return envelope and seal the envelope.
10. Dispatch the envelope immediately on the same day. There are 3 shipping options:
    - Deposit the envelope at your GP (best option).
    - Deposit the envelope at a Star-shl location.
    - Ship the envelope with PostNL free of charge? Stamp not needed.
    Store the envelope in the refrigerator if you won’t be able to dispatch immediately.
    Note: If blood samples are also required, you will receive an additional request form separately. In that case you can deposit the tubes at a blood drawing location together with the envelope.
11. Your GP will usually receive results within 2-3 working days after the laboratory received the envelope. Your GP will discuss the results with you.
